JOB DESCRIPTIONThe Patient Care Assistant supports physicians and nursing staff in meeting the basic needs of patients and ensuring their comfort and safety. Responsibilities include assisting with daily living activities, monitoring and recording vital signs, and supporting care coordination.
Key Responsibilities:
- Assist patients with activities of daily living feeding, dressing.
- Obtain and record vital signs, weights, and heights.
- Support patient mobility, including ambulation, transfers, discharges, and range-of-motion activities.
- Perform EKGs, phlebotomy, and blood glucose monitoring.
- Observe and promptly report changes in patient condition to the RN.
- Maintain phlebotomy equipment and supplies.
- Assist clinicians with care coordination, including communicating with primary care providers or other healthcare professionals (with patient consent).
- Collect specimens as assigned (e.g., urine, saliva) when applicable.
- For group therapy settings: assist with or conduct psychoeducation and activity groups, ensure a safe environment, support patient management and de-escalation, and maintain cleanliness of care areas.
- Apply infection control and safety standards in all work activities.
- Perform other related duties as assigned.
QUALIFICATIONS
Qualifications:
Required:
- High school diploma or equivalent.
Preferred:
- Strong verbal and written communication skills.
- Compassionate and patient-centered approach.
- Ability to work collaboratively in a team setting with physicians and nurses.
- Proficiency in computer-based data management and information systems.
