The Position
The Director of Care Coordination will be responsible for overseeing care coordination services at MercyOne Medical Center and MercyOne West Des Moines Medical Center, ensuring that organizational goals and operational responsibilities are consistently met.
Reporting directly to the Chief Medical Officer, this leader will oversee two direct reports and 35 FTEs.
Key priorities include improving Length of Stay within the first 90 days of hire, reinvigorating performance outcomes, and establishing clear processes and structures for managing long-term and complex patients.
Ideal candidates will be recognized subject matter experts in care coordination, with experience spanning both inpatient and outpatient settings. This role calls for an inspiring leader with a track record of driving engagement, unity, and a best-in-class work environment. A strong history of delivering results and driving successful organizational change is critical.
Requirements
Bachelor’s degree required; Master’s degree or active enrollment in a Master’s program required.
Minimum of five years of leadership experience in a large, complex setting at the Director level or equivalent, along with at least three years of relevant care coordination experience, is required.
